The ocular prosthesis is subject to a medical prescription.

Your medical prescription must be issued by your ophtalmologist following an appointment. 

Prescription valid for one year.

(see Social security coverage file above).

On these documents it must appear the number of the identifier of the ophthalmologist and the type of the prosthesis
(all pages must imperatively be stamped and signed).

For your first appointment, provide your current and valid vital certificate and your medical prescription.

If all these conditions are met in your file, we can perform the third party payment.
